HC PrefNote is developed and published by Zendi Labs LLC. If you have any questions about these Terms, contact us at support@zendilabs.com.
We grant you a personal, non-exclusive, non-transferable, revocable license to use HC PrefNote on devices you own or control, solely for its intended purpose of creating and organizing provider and surgeon preference notes.
HC PrefNote Pro is offered as an auto-renewing subscription, available as Monthly or Annual plans, each beginning with a 30-day free trial.
Payment is charged to your Apple App Store or Google Play account upon confirmation of purchase. Subscriptions automatically renew at the price shown above unless canceled at least 24 hours before the end of the trial or current billing period. Your account will be charged for renewal within 24 hours prior to the end of the current period. You can manage or cancel your subscription at any time in your App Store or Google Play account settings. If you cancel during the free trial, you retain full access through the end of the 30 days and will not be charged.
HC PrefNote is intended for use as a personal preference card reference tool by healthcare professionals. It is not a medical device and does not replace clinical judgment, institutional policy, or official preference card systems. You must not store patient names, medical record numbers, dates of birth, diagnoses, or any other protected health information (PHI) in the app.
All preference cards and data you create remain on your device. You are solely responsible for the content you create, import, or share using the app, including data you export or transmit to other users via file sharing or QR code.
HC PrefNote is provided "as is" and "as available" without warranties of any kind, whether express or implied. We do not warrant that the app will be uninterrupted, error-free, or that it is suitable for any particular clinical or institutional purpose.
To the maximum extent permitted by law, Zendi Labs LLC shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of, or inability to use, HC PrefNote.
We may suspend or terminate your access to the app if you violate these Terms. You may stop using the app and delete it at any time.
We may update these Terms from time to time. Any changes will be posted on this page with an updated date. Continued use of the app after changes constitutes acceptance of the updated Terms.
Your use of HC PrefNote is also governed by our Privacy Policy.
